How to report an incident

Gather the information you want to share, then complete the incident report.

1. Describe the incident

   Select the state, enter the local government and community, and explain what happened.

2. Add the location

   If you are at the incident location, choose “Capture my location”. Your browser will ask for permission. You can still report using the location details you enter if GPS is unavailable.

3. Attach a picture or video

   Choose a file from your device, up to 4 MB. On supported phones, the file picker also lets you use your camera.

4. Complete your contact details and submit

   Enter your personal information and select “Submit report”. Keep the page open until your report is confirmed. If submission fails, your entries remain available to retry.
